Surgical rehabilitation for the weaker patients (Groups 1 and 2 of the International Classification)

This article will deal with those patients graded group I and group 2 according to International Classification (IC). These patients typically display relatively normal function in the deltoid and biceps muscles. Group I patients also have a strong brachioradialis muscle (graded Medical Research Council (MRC) 4 or above), and group 2 patients have an additional MRC grade 4 muscle, typically the extensor carpi radialis longus (ECRL). The triceps is usually absent or weak in these high-level groups. In the ASIA (American Spinal Injury Association) system, these patients are classified as either C5 or C6 level. These two groups of patients represent a relatively large proportion of the overall tetraplegic population. In the larger tetraplegia series [1-3], IC groups 0 and I are often added, and amount to 13 to 54% cases, and IC group 2 accounts for 13 to 48% cases. In our series of operated patients, IC group 1 represents 25% upper limbs, and IC group 2 represents 6% upper limbs. Differences in rates probably result partly from differences in interpreting the International Classification.
