Body, head and limb posture are supposed to be of great importance for the distribution pattern of spasticity and weakness in hemiparetic patients. The present study investigates the influence of three different postural conditions on transcranially evoked motor potentials in the biceps and the abductor pollicis brevis muscles of 32 stroke patients. An effect induced by the actual body or limb position was not detectable. This makes the significance of this view appear doubtful in traditional physiotherapeutic concepts, particularly in the Bobath concept. Instead of spending a lot of time on manipulating head, trunk and proximal limb positions physiotherapists are encouraged to address centrally paretic muscle groups directly - even at an early stage of recovery.
