Injuries to the lateral column of the elbow can vary widely in severity and management may often be difficult and controversial. Management of radial head fractures has evolved over the years, as the radial head's role in elbow and forearm stability has been elucidated. Currently, preservation or replacement of the radial head is generally advised whenever possible. Other management issues, such as duration of immobilization, preoperative planning, and surgical technique have changed as our knowledge of complex elbow pathology has increased. Radial head fractures compose the vast majority of lateral column fractures because they are the most common elbow fracture. Capitellum fractures are much less common, but result from similar mechanisms and present a unique management challenge. Concomitant fractures have a variable incidence, and it is suspected that they are frequently missed.
