Acetabular femoral interactions in the loosening of total hip endoprosthesis

One-hundred and one primary Christiansen total hip replacements were subjected to a radiological and clinical analysis after a postoperative period of 15 to 93 months (mean 40 months). The follow-up period ended at the time of reoperation for loosening in 18 cases. Infected cases (4%) were excluded. Several associations were found between aseptic acetabular and femoral loosening signs. The most interesting finding was the concurrent highly significant association between wide acetabular and femoral radiolucencies in the cement bone interface. Definite signs of femoral loosening (migration and/or fracture of the Methylmethacrylate bonding) were also associated with acetabular loosening signs. We conclude that stresses in the whole prosthesis system represent an important factor in component loosening.
