It can cause back and leg pain, neurological deficits, musculoskeletal deformities, and bladder disturbances and each of these manifestations is potentially correctable if the diagnosis is made promptly. We review the embryogenesis, biomechanics, clinical presentation, diagnostic evaluation, and treatment of the tight filum terminale, and we explore the role of tethering in the pathogenesis and management of other related congenital spinal lesions.
