Measuring disability in terms of physical abilities only is no longer appropriate when definitions of disability include social and psychological dimensions and rehabilitation focuses on incorporating users' need into service design and evaluation. A critical review of the Barthel Index as an exemplar of physical measures is presented with alternative approaches discussed.
