The lack of peripheral sensation and proprioception can be the causal factors in the development of a pedal stress fracture. A case is presented of a second metatarsal stress fracture in a patient with peripheral sensory neuropathy and peripheral autonomic neuropathy secondary to diabetes mellitus. The literature relative to this entity is reviewed.
